The *Therapist* interacts with the treatment team and provides a variety of patient care activities daily.
They are responsible for managing patient caseload, providing group-individual-and family therapy, conducting bio-psychosocial assessments and more! The Therapist collaborates with the treatment team on a daily basis, formulates the master treatment plan and assists in coordinating discharge planning while functioning under the guidance of their credentials and State/Federal regulations.
*Essential Functions:*
* Responsible for client caseload
* Provides group therapy, daily process group.
* Provides individual and family therapy.
* Formulates client's master treatment plan
* Responsible for documenting progress notes and clinical interactions into client EMR
* Completes discharge paperwork in EMR in accordance to facility requirement
* Conducts bio-psychosocial assessments
* Collaborates with the treatment team in other departments
* Attends clinical meetings
* Acts in accordance with the regulations, rules, and requirements of State and Federal government and accrediting organizations.
*Qualifications:*
* Master’s degree in social work or a related major.
* CADC
*Preferences:*
* Work experience in the field of behavioral or mental health.
* LPC, LSW, LCPC or LCSW in the state of IL.
