The Family Advocate provides advocacy and support services to Hammitt School students and their families that promote and enhance the overall academic mission by providing services that strengthen home, school, and community partnerships and address barriers to learning and achievement.

JOB RESPONSIBILITIES OF THE FAMILY ADVOCATE INCLUDE:
Assessment: Evaluates student behaviors in the home and parenting skills, determines strengths and needs, and develops data to share with the clinical team.
Service/Treatment Planning: As part of the interdisciplinary team provide data to assists in the development, monitoring, reviewing, and modifying of Individualized Education Programs (IEP’s) for assigned students throughout period of service provision.
Advocacy, and Support: Through an interdisciplinary team process, provides advocacy, and support by:
Serving as the liaison between home and school to share information and assist in improving student attendance, behavior, and achievement in school; stabilizing the child’s emotional functioning in both the home and school settings; and engaging both the child and family in activities and support services that will enhance school success.
Meeting with families when and where it is most convenient for them
Providing referrals to and liaising with community resources, medical providers, and schools on behalf of students and families.
Providing parent education on topics such as child development, disabilities, parenting techniques and interventions, and behavior management.
Team Responsibilities: Maintains formal and informal communication with Hammitt School staff to ensure that information concerning the students’ adaptive behavior, social, cultural/ethnic and environmental factors, and family system are considered in the educational planning process, including transition from the Hammitt School program.
REQUIREMENTS TO BE CONISDERED FOR FAMILY ADVOCATE:
Bachelor’s Degree in social work or a related field required; Master’s degree in social work or related field is preferred.
Knowledge of child growth and development, behavior, and child management necessary to apply in the educational and home setting.
Willingness and ability to work evening and weekend hours as needed to fulfill job responsibilities and to meet the needs of the families we serve.
Must travel for job responsibilities, including the ability drive distances to enter the homes of families in the service area and throughout Central Illinois, a valid Illinois driver’s license that has not been revoked or suspended within the past three years, and use of a personal vehicle with adequate insurance.
